Today, Via Christi Health is the largest provider of health care services in Kansas.Position Summary:Serves as the professional medication expert resource at Via Christi for physicians, nurses, patients and other employees. Provides optimal patient safety through knowledge and skill applied to the drug distribution and clinical responsibilities of the position. Assists in meeting the business needs of the organization by practicing the core values and following policies, laws and regulations.Key Responsibilities/Essential Functions: Reviews all appropriate patient information (e.g.
allergies, medication history, pertinent lab values, drug interactions, therapeutic duplication) prior to profiling or verifying new medication order(s). Reviews each medication order before profiling or verifying it to ensure that the patient will receive the correct drug, dose, route, frequency and monitoring that is individualized for them, also uses IT-related tools (e.g. PCOs---predefined common orders) and order sets during the order profiling process. Actively participates in the clinical programs and clinical monitoring/intervention expectations of the pharmacy department and hospital, which includes knowledgeable review of clinical reports and alerts, and the provision of targeted clinical services as defined in current department training.
Provides appropriate, timely and accurate medication-related information to physicians, nurses and other healthcare professionals that are patient-specific when appropriate. Provides guidance, information and direction to pharmacy technicians and support personnel in order to safely and efficiently complete all required workload tasks, while carefully checks all work of pharmacy technicians as required by policy, law and regulation. Utilizes approved formulary drugs in the order profiling and drug distribution process, including therapeutic interchanges as approved by the Medical Staff, as well as recommends a formulary therapeutic alternative, when appropriate, to the prescriber. Accurately and completely documents work in a timely manner as required by department training, such as order clarifications, therapeutic interchanges, clinical interventions, medications checked prior to dispensing, liaison inspections, department competencies, and orders requiring a double-check.
Actively pursues enhancement and improvement of self-knowledge of medication therapy, department policies/programs, and new/ongoing initiatives through completion of department competencies, didactic programs, in-service educational opportunities, staff meetings, continuing education programs, etc. Participates in continuous performance improvement activities and processes that contribute to the growth and improvement of the individual and also contribute to enhancement of the services provided by the pharmacy department Follows all department, hospital and regulatory agency policies, laws, regulations and requirements. May perform other duties as assigned or requested. JOB REQUIREMENTS: Qualifications:Education and/or Experience Bachelor's degree (5 year minimum) or Doctorate degree in Pharmacy from an approved school of pharmacy, preferred.

Certificates, Licenses, Registrations Requires licensure as a registered pharmacist in the state of Kansas; eligibility for licensure is acceptable, but employee must obtain Kansas licensure within 90 days of employment. ACLS as designated by management.
