Background and-or Security Investigation required DUTIES:
The Clinical Pharmacy Specialist practices in a major clinical area (primary care, psychiatry, medicine, geriatrics, home based primary care, etc.) in the health care organization. The incumbent functions independently as a mid-level practitioner and works with assigned providers and health care team(s) in designing, implementing, monitoring and evaluating, and modifying patient pharmacotherapy. Duties include, but are not limited to, the following: Evaluating and monitoring medications for appropriateness of therapy and patient's response to therapy based on relevant therapeutic endpoints by means of patient interviews, physical and assessment, and diagnostic tests.
Requesting consultations or referrals to other health care professionals to render appropriate comprehensive care to t! he patient.
Prescribing and modifying medication regiments in a saf! e, effective, and economical manner.
Providing patient/caregiver education
Documenting interventions and care in the patient's medical record.
Serving as the clinical manager of the pharmaceutical care programs on the outpatient and inpatient care areas
Implementing and monitoring formulary management protocols and programs approved by the Pharmacy and Therapeutics Committee.
Managing and providing multiple complex medication management programs that are applicable to the patient population of their assigned health care team(s).
Supervising and providing training to pharmacy students and pharmacy residents.
Designing and/or participating in research studies that have under went review and approval by the Research and Development Committee and the Investigational Research Board.

Preferred Experience: Preferred candidates will have a have Doctor of Pharmacy degree, completed by an accredited pharmacy residency with emphasis in ambul! atory care and/or have extensive practice experience providing advanced medication therapy management.

Tour of Duty: Primarily
Monday-Friday Day Shifts. Incumbent will be required to work non-administrative
tours of duty including weekends, evenings, and holidays based on the

needs of the organization. QUALIFICATIONS REQUIRED:BASIC REQUIREMENTS:
U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduate of an Accreditation
Council for Pharmacy Education (ACPE) accredited college or School of Pharmacy
with a baccalaureate degree in Pharmacy (BS Pharmacy) and/or a Doctor of
Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet
the educational requirement if the graduate is able to provide proof of
achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC)
Certification, which includes ! passing the Foreign Pharmacy Graduate Equivalency
Examination (FPGE! E) and the Test of English as a Foreign Language Internet-Based
Test (TOFEL iBT). Transcripts Required
Full, current and
unrestricted license to practice pharmacy in a State, Territory, Commonwealth of
the United States (i.e., Puerto Rico), or the District of Columbia. Copy
of current license required.
Must be proficient in spoken
and written English as required by 38 U.S.C. 7402(d) and 7407(d).

In addition to the basic requirements listed above, all applicants must meet the guidelines as outlined in VHA Directive 2009-014, Establishing Medication Prescribing Authority for Clinical Pharmacy Specialists, dated March 12, 2009 ( http://www1.va.gov/vhapublications/ViewPublication.asp?pub_ID=1852 ). Applicants must have prescriptive authority and possess: a current state license and Pharm.D. or M.S. degree (or equivalent, i.e., completion of an American Society of Hospital Pharmacist accredited residency program, Specialty Board certification! or 2 years of clinical experience). Applicants who will complete their residency training program by June 30, 2013 will be eligible given that they meet all other basic requirements.

GRADE DETERMINATIONS:
To qualify at the GS-13 level, in addition to
the basic requirements listed above, applicants must have 1 year of experience
equivalent to the next lower grade level. In
addition, pharmacists at the GS-13 level must also demonstrate the following

Knowledge, Skills and Abilities (KSAs):
Ability to communicate orally and in writing to persuade clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertis! e.
Expert knowledge of a specialized area of clinical pharmacy prac! tice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
