Qualifications we are seeking for this position are:
Ø Registered Pharmacist Licensure in appropriate state(s)
Ø Doctor of Clinical Pharmacist (PharmD) preferred
Ø Board Certification as a Geriatric Pharmacist (CGP) required or must be willing to obtain this certification within the first two years of employment â€" company paid!
Ø Experience in geriatric pharmacy, preferred
Ø Must be able to travel with some overnight stays â€" Up to 50% travel
Ø Basic understand of Microsoft Office products, preferred
Knowledge and Experience we are seeking :
Ø Possess thorough knowledge/experience of positive medication related practices and outcomes.
Ø Able to analyze situations, manage associated risk by monitoring, discontinuation ! of medications, or changes to safer alternatives.
Ø Able to effective serve as liaison between the contracted provider pharmacy and living center to monitor for, act on and resolve any pharmacy related issues.
Ø Must be able to promote and educate all aspects of safe use, associated disease states, regulatory requirements to living center-level staff.
Ø Must be able to perform CMS required Medication Regimen Reviews as required by law
Ø Must be self-motivated, organized and detail-oriented and require little supervision.
Ø Ability to maintain confidentially regarding patient and company proprietary information
Ø Able to relate professional and positively and work cooperatively with other employees and healthcare team members at all levels.
