Detailed specification about this career opportunity please read the description below. Via Christi is offering an exciting opportunity for a motivated Ambulatory Care Clinical Pharmacist. This individual will assist in the initiation and expansion of pharmaceutic! al care services to patients across our health-system through our Patient Centered Medical Home (PCMH) clinics as well as Via Christi Health Plan Participants. With an initial focus on improving patient outcomes via the provision of MTM services including the following conditions (diabetes, asthma, COPD, hyperlipidemia, and hypertension), the pharmacist will serve as a key member of the healthcare team and work alongside providers under a collaborative practice agreement. The successful candidate will have excellent communication and presentation skills as well as knowledge and/or experience in developing new ambulatory care pharmacy services.
Involvement in education of physicians, medical residents, pharmacists, pharmacy residents, pharmacy students, nurses, and other healthcare professionals is a key responsibility. Opportunities for adjunct faculty appointment and the ability to conduct clinical based research are available and encouraged.
