The Wellness and Condition Management team conducts annual Wellness Screenings, utilizing an innovative, holistic approach, and offers Condition Management programs to associates of Cerner. This position is located in Kansas City, KS at the new Continuous Campus Clinic scheduled to open in Jan 2014.
During the Wellness Screening, data from an on-line Personal Health Assessment (PHA) is coupled with lab results/vitals data and claims information to create a snapshot of a member's health. Cerner's rules engine then creates a risk assessment, which serves as the framework for a one-on-one discussi! on between the advisor and the member. The Advisor will help i! nterpret key data points and helps the member set goals toward overall health improvement.
During the Wellness Screening the Wellness Coach:
Administer blood draw and capture biometrics for individual members.
Review and help the member interpret key data points in the wellness screening (PHA, labs, vitals, recommendations).
Refer members with at risk or borderline data results to their PCP.
Helps the member to set goals toward their overall health improvement.
Serve as a link to members and the various services offered. These benefits and services include, but are not limited to: Condition Management Programs, Exercise Programs, Nutritional Services, getting access to an appropriate physician, and understanding medical benefits.
The team also offers a number of Condition Management programs to members. The Wellness Coach is also responsible for actively coaching members with chronic conditions towards a healt! hier state of being. This includes:
Participate in a coordinated care team model. This will include a physician, MA, nurse and coach. The team will work very closely on a daily basis to provide outstanding care to members. (aka âteamletâ model)
Coach a member on a continuum of risks (ranging from single risk factor, to pre-condition status, to diagnosed condition status)
Performs individualized assessments, including the member's readiness to change, and develops unique plans of care for each member.
The Wellness Coach must be capable of utilizing multiple interventions including educating, advocating, guiding, and listening.
Responsible for evaluating the effectiveness of interventions and collaborating with other health professionals as needed including pharmacists, dieticians, and healthcare providers.
Qualifications
Minimum Qualifications:
All current and previous licensures in good standing
Three (3) yearsâ experience in the field(s) of case managemen! t, ambulatory care, home health, discharge planning, wellness coaching, and nutrition
Demonstrated knowledge of managed care principles, computer programs and systems (Excel, Word, Microsoft Office Products)
Knowledge of current development and advances in the care of chronic diseases and preventative health
Ability to work as a member of a team and collaborate with a variety of individuals
Excellent verbal and written communication skills
Presentation and classroom teaching skills
Strong organizational/time management skills
Willingness to learn new software
Knowledge of HIPAA and patient confidentiality
Must reside in or be willing to relocate to the Kansas City metropolitan area
Ability to work flex hours -- some early morning and some evenings as needed
Preferred Qualifications:
Bachelor's degree in health related profession
Motivational communication s! kills and presentation/training skills
knowledge of health behavior change principles and motivational interviewing techniques
Demonstrated ability to develop program content, educational materials and evaluation tools
