Via Christi is offering an exciting opportunity for a motivated clinical pharmacist interested in serving solid organ transplant patients ! / program. This pharmacist will be integrally involved in the provision of comprehensive pharmaceutical care services to transplant program patients, working closely with physicians, nurses and other members of the team to optimize patient and program outcomes. This interdisciplinary work will help ensure high reliability, patient safety and patient satisfaction. Direct patient care services will be provided in both the inpatient and ambulatory settings, helping to ensure continuity and excellent patient outcomes.
The successful candidate will have excellent communication and presentation skills, be able to successfully implement change at both the patient and group levels, and be able to work well with providers from various practice specialties. Involvement in education of physicians, medical residents, pharmacists, pharmacy residents, pharmacy students, nurses and other healthcare professionals is a key responsibility.
